Last updated 4 September 2026. Daisy Hill Physiotherapy (“we”, “us”) is committed to protecting your personal information in accordance with the Privacy Act 1988 (Cth), the Australian Privacy Principles and the relevant Queensland health-records requirements.

What we collect

We collect personal information you provide to us, including your name, contact details (phone, email, address), date of birth, funding details (for example NDIS, DVA, WorkCover, CTP or Medicare care-plan information), and health information relevant to your physiotherapy care. When you use our website, landing pages or online forms we may also collect the information you enter, and standard technical data such as your device type and the page you came from (including advertising campaign identifiers).

How we use it

  • To contact you about your enquiry, book and manage appointments, and provide physiotherapy and hydrotherapy services.
  • To check and process funding claims with the relevant insurer, scheme or Medicare.
  • To send appointment reminders, service information and, where you have opted in, marketing about our services. You can opt out at any time by replying STOP to an SMS, using the unsubscribe link in an email, or contacting us.
  • To improve our website and advertising, including measuring the effectiveness of ads on platforms such as Meta (Facebook/Instagram) and Google.

Who we share it with

We share information only as needed to deliver your care and process your enquiry: your treating practitioners, your GP or specialist where you have consented, funding bodies and insurers for claims, and the service providers that operate our practice systems (practice-management software, secure messaging, email and SMS providers, and automation tools). These providers may store data in Australia or overseas and are required to protect it. We do not sell your personal information.

Online enquiry forms and advertising

If you submit an enquiry through one of our online forms (including forms reached from a Facebook or Instagram ad), the details you enter are sent securely to our reception team so we can call you back. Meta and Google may collect information about your interaction with our ads under their own privacy policies.

Storage and security

We take reasonable steps to keep your information secure, including access controls, secure practice-management software and staff confidentiality obligations. Clinical records are retained for the periods required by law.

Access and correction

You may request access to, or correction of, the personal information we hold about you by contacting us using the details below. We will respond within a reasonable time.

Complaints

If you have a concern about how we have handled your information, please contact us first. If you are not satisfied with our response you can contact the Office of the Australian Information Commissioner (oaic.gov.au).
