A 5-visit Medicare CDM allocation isn’t a lot — so every session has to move the needle. We use the first appointment to build the assessment and the program, then the remaining 4 to load, progress and review against measurable goals. You leave the 5 visits with a self-manageable program, not a dependence on more sessions.

How to pay for Medicare CDM physiotherapy
Medicare CDM rebate
Medicare rebates a fixed amount per Allied Health visit under a Chronic Disease Management plan. You pay the gap to the consult fee.
How to qualify
You need a GP-issued CDM plan (form previously known as GPMP/TCA, now CDM). Your GP confirms a chronic condition and writes a referral for physiotherapy.
5 visits per year
Across allied health (physio, podiatry, dietetics combined) — not 5 of each. Most patients prioritise physiotherapy.
Outside the 5 visits
After your CDM allocation we continue under private fee or HICAPS-claimable private health rebate. No gap in care.
