Who this service is for
This page is for aged-care facility managers, clinical care coordinators, directors of nursing, retirement village operators, and home-care package providers in the Logan and south Brisbane area who need a reliable, locally-based physiotherapy partner.
What we deliver on-site at your facility
- AN-ACC-aligned individual physiotherapy assessments — required documentation to support resident classification and funding
- Falls-prevention programs — group and individual, evidence-based, integrated with your existing care plans
- Mobility maintenance — programs to slow functional decline in long-term residents
- Post-acute and post-surgical rehabilitation — supporting residents returning from hospital admissions
- Pain management — manual therapy and physiotherapist-prescribed exercise for chronic pain in older residents
- Manual-handling and transfer training for care staff — reducing staff injury risk and improving resident safety
- Hydrotherapy referral pathways — for residents who would benefit from pool-based exercise
- Written reporting and progress notes — compatible with your clinical record system; supports quality, accreditation and care planning
Why partner with Daisy Hill Physio
- Locally based in Daisy Hill — short travel time means more clinical hours, less travel charge, faster response to ad-hoc requests
- Established 1987 — long-standing local clinical reputation, strong GP network across the Logan corridor, registered with AHPRA and APA-recognised
- Multidisciplinary capability — physiotherapy, vestibular, women’s and men’s pelvic health, hydrotherapy and clinical Pilates all under one provider
- NDIS and DVA registered — supports residents accessing those funding streams in parallel with facility-funded care
- Consistent clinician allocation — wherever possible we allocate the same physiotherapist to your facility long-term, so they know your residents, your care team and your processes
Service delivery model
We typically build a contract around a regular on-site visit (weekly or fortnightly) with an allocated physiotherapist, scaled to the size and complexity of your resident base. Initial AN-ACC assessment blocks can be scheduled separately on shorter timelines. We invoice the facility monthly on agreed rates with clear time-and-activity reporting.
Funding alignment
Our on-site service can be funded through any combination of AN-ACC physiotherapy supplements, facility operating budgets, individual Home Care Packages (for retirement village residents), Medicare GPCCMP for eligible residents, and DVA for veteran residents. We can support your billing team to map services to the most appropriate funding source on a case-by-case basis.

Frequently asked questions — facility services
Do you work with retirement villages as well as RACFs?
Yes — we work with both. For retirement village residents we typically deliver through their individual Home Care Packages or under Medicare/DVA pathways.
Can you scale up for a larger facility?
Yes. We allocate additional clinician time based on resident numbers and complexity. We can also bring in additional clinicians for assessment blocks (e.g. AN-ACC reassessment periods).
Are your clinicians AHPRA-registered and APA members?
Yes — all of our clinicians are AHPRA-registered and members of the Australian Physiotherapy Association.
Do you carry your own professional indemnity and public liability?
Yes — current certificates of currency available on request.
How quickly can you start?
For a typical contract, we can begin on-site service within 2–4 weeks of agreement, including induction and contractor onboarding requirements at your facility.
